Thank you for sponsoring the 2023 Caregiver Recognition Event! Please use this form to declare your sponsorship level, recognize a caregiver(s), and list your guests. You will need contact information for each person. DO NOT use your agency/organization’s contact information. If you are recognizing a caregiver(s), you will need to submit a maximum 500-word narrative describing what makes them an exceptional caregiver. Also include 1-2 photos and your company’s logo (in JPG or PNG image format).
SPONSORSHIP CHECKLIST
    1. Review the Sponsorship Categories listed below to determine your level of sponsorship.
    2. Review the Caregiver Categories listed on the 2023 Caregiver Submission page to determine which category best fits your caregiver.
    3. Click on the button at the end of the page “Become a Sponsor!”.
    4. Complete the Sponsorship Form, register yourself and your guests, then add information about any caregiver you are recognizing.
    5. Make your credit card payment.
NOTE: If you want to register more people for the luncheon than your sponsorship provides, please do so through the general registration page. Each additional guest will cost $40.
